Middleton Counseling and Consulting
Sarah Middleton LPC (They/Them)
I provide referral letters for medical transition surgeries. Typically, a single session is conducted for the letter, and there's no charge for the session or letter. I follow an informed consent model, believing individuals are best equipped to make decisions about their medical needs in collaboration with a medical provider. Clients seeking these services usually have a good understanding of the risks, benefits, and alternatives of their chosen procedure before reaching out. If you need more assistance in determining the right transition path, please inform me before scheduling.

Gender Affirming Hormone Treatment and Surgery Evaluations and Letters
I have been writing these letters for over 4 years and have over 2500 direct clinical hours working with the transgender community. Evaluations are 60 - 90 minutes via telehealth and cost $150 though I take a variety of insurance. You will get a copy of your letter and I will fax it to the physician of your choice. Letters are usually ready within 2 business days of the evaluation. I follow WPATH standards. I am very passionate about what I do and helping you get the surgery and hormones you have been wanting and needing.
